MODULE 1

This course begins with an overview of the U.S. health care delivery systems. The U.S. health care system is influenced by the following forces:

Internal Factors:

Financing:

· Public health insurance programs such as Medicare, Medicaid, State Children’s Health Insurance Program, and TriCare.

· Employer-provided health insurance.

· Commercial health insurance.

· Out-of-pocket payments.

Payment:

· Payment mechanisms used by public and private health insurance programs, including but not limited to fee-for-service, capitation, per diem, global budget, pay for performance, shared savings, gain sharing, bundled payments.

Delivery:

· Health care delivery institutions such as hospitals, ambulatory care clinics (e.g., community health clinics, and physician offices), pharmacies, and rehabilitation centers.

Resources:

· Health care human resources, including all types of health care professionals (physicians, nurses, therapists, pharmacists, technicians, and so on), as well as management and administrative personnel.

· Health IT infrastructure.

· Medical equipment.

Regulation:

· Federal and state legislation that regulates all agents in the health care system (e.g., Patient Protection and Accountable Care Act [PPACA], Health Insurance Portability and Accountability Act [HIPAA], Consolidated Omnibus Budget Reconciliation Act [COBRA], and Employee Retirement Income Security Act [ERISA]).

External factors:

· Economic development.

· Political climate.

· Population characteristics such as demographic composition and health outcomes.

· Social and cultural values.

· Technological development.

· The combined interaction of these forces influences the structure and the development of the U.S. health care system.

Major Characteristics of U.S. health care delivery:

· No central governing agency to integrate and coordinate health care.

· High in cost, unequal in access, and average on outcome.

· Access to health care services is selectively based on insurance coverage.

· Technology-driven delivery system focusing on acute care.

· Legal risks influence practice behavior.

· Multiple players and balance of power.

· Quest for integration and accountability.

Please note that there is a quiz as part of the Module 1 Discussion that must be successfully completed, and certificate submitted, before you can progress to the Case and SLP assignments in Modules 2-4.

Course Guidelines:

1. Cover Page:

Use a cover page on all the assignment papers you submit. Do not include the text of the assignment on your cover page; simply your name, course number and session, date, and assignment title will suffice. Read the assignments carefully and make sure you respond to each part (I recommend that you print/highlight each required item as you successfully include it in your papers).  Strive for the number of pages noted in the assignment’s expectations – no more, no less.

2. Writing Style and Format:

Graduate Student Writing Handbook –This is an excellent resource for help in constructing graduate-level papers as well as also ensuring that these papers are original.  I encourage you to utilize this resource and if you have questions, please do not hesitate to contact me.  The handbook can be downloaded at the following link: https://mytlc.trident.edu/files/GRADUATE_WRITING_HANDBOOK.pdf

All students are required to use a standardized format of citations and references. Trident encourages, but does not require students to follow APA style. As an example: Author, A. A., & Author, B. B. (yyyy). Title. Journal. The OWL is an excellent resource for APA style and I highly recommend it.  https://owl.english.purdue.edu/owl/resource/560/01/

3. Implementation of Feedback:

Please wait for my feedback for your Module 1 assignments before submitting Module 2. This will help ensure that you are on the right track, and can implement my comments in your subsequent work.  It is best to send your Case, wait for feedback, then send your SLP (so any corrections can be made to that).  This may help your grade!

·  Case

· Evaluate the U.S. health care system and employ critical thinking to fractionize trending assumptions.

· SLP

· Differentiate the U.S. health care delivery system from other industrialized countries’ health care delivery systems.

· Discussion

· Appraise the impact of organizations, clinicians, and patients on health care delivery.

Module 1: U.S. Health Care Delivery Systems Discussion: Integrity: Key Players

1st part of assignment. Discussion: Integrity: Key Players

No more than a 1 page discussion of the below topics.

As a leader, one of the many qualities that you must embrace during your journey, as a professional or a student, is integrity. According to Telford & Gostick (2005), “integrity is tough taskmaster… the ability to consistently act in accordance with their own principles is one thing that sets people of integrity apart” (pg. 1). As we begin this education journey, let’s take a moment to review aspects of integrity in the classroom. Begin by reviewing and completing the graduate tutorial at the following source.

Frick, T., Dagli, C., Barrett, A., Myers, R., Kwon, K., & Tomita, K. (2019). How to recognize plagiarism: Tutorial and tests. Bloomington, IN: Department of Instructional Systems Technology, School of Education, Indiana University. Retrieved from: https://academy.sitehost.iu.edu

Once you have completed your tutorial, you will receive a certificate. Submit the Certificate in your Case 1 Assignment dropbox (Note: you can submit multiple attachments). Please note that you MUST complete this tutorial, successfully pass the quiz, and submit your certificate before you can progress with Case and SLP assignments beyond Module 1.

There are three key players in the delivery of health care: 1) the health care organization, 2) the clinician (team of physicians, nurses, medical assistants, and office staff), and 3) the patient (Coqing, Daviono-Ramaya, Ramaya, & Szmerekosky, 2009). For Module 1 Discussion, discuss the influence each key player has on healthcare delivery and explain the importance of integrity for each player.

Your posts will be graded on how well they meet the Discussion Requirements posted in the “Before You Begin” section. Please review this section as well as the discussion scoring rubric.

PART 2, CASE ASSIGNMENT############

Module Overview

Health Care Delivery in the U.S.

Module 1 – Case

U.S. HEALTH CARE DELIVERY SYSTEMS

Assignment Overview

The United States has a unique system of health care delivery which has evolved over time. Although the U.S. health care delivery system has evolved in response to cost, access, and quality, there are often assertions made about health care delivery in the United States. For example, according to Mary Mahon (2015), VP of Public Information at the Commonwealth Fund, the U.S. spends more on health care than other high-income nations, but has a lower life expectancy and worse health.

Case Assignment

After completing the assigned readings, you should have a basic understanding of the characteristics and components of the U.S. health care delivery system. For the Module 1 Case Assignment, review the following assumptions regarding health care in the U.S. (Greenwald, 2010). Although these assumptions are widely publicized, they might be disputed with critical thinking. Evaluate additional literature and consider each of the following assumptions. Explain why you believe the statement is true or false.

· The U.S. spends too much on health care services.

· The poor lack health care.

· The systems in other countries are better.

· The U.S. health care system is in crisis today.

· Health care is a right.

Assignment Expectations

1. Provide a one-paragraph (5- to 7-sentence) response for each statement.

2. Include an introduction and conclusion in your paper.

3. Support your report with peer-reviewed articles, with at least 3 references. Use the following link for additional information on how to recognize peer-reviewed journals. Discussion: Rural vs. Urban Ancillary Services

Discussion Topic

Task: Reply to this topic

Health care services vary given a geographical location of an organization. All services may not be provided in one area; furthermore, patients who live in rural areas must often travel to obtain services. For example, according to Spasojevic, Vasilj, Hrabac, & Celik (2015), rural residents are more likely to travel more than 15 minutes to see their health facilities compared with urban residents. Based on the course readings and your own research, discuss/answer the following:

1. What are ancillary services? (Provide 2 or 3 examples.)

2. How do ancillary services offered in rural and urban areas differ?

3. Regarding ancillary services, what are some disadvantages to consumers (patients) living in rural populated areas?

4. What recommendations would you provide as a health care leader to lessen these disadvantages?

Your posts will be graded on how well they meet the Discussion Requirements posted in the “Before You Begin” section. Please review this section as well as the discussion scoring rubric.
